Requesting Vital Records
The Town Clerk's Office has vital records for the Town from 1640 to the present. To request a birth, death, or marriage certificate by mail use the Request for Vital Record document (PDF).
Complete the appropriate section on the form and mail together with a check for $10 for each certificate requested to the address listed above. Please include a stamped, self-addressed return envelope.
Certified Birth Records
The town will have the birth record of any person whose parents resided in Braintree at the time of the child's birth.
The Town of Braintree issues only the "long form" birth certificate. This certificate contains all information at time of birth (parents, place of birth, physician, etc.) and is needed for passports, insurance purposes, etc.
Certified Marriage Records
The town will have the marriage record for any couple who filed their marriage intention (marriage license application) with the town.
Certified Death Records
The town will have the death record for any person who passed away in that town or was a resident of the town at time of death.

Please fill out the Request for Vital Record form (PDF), submit with appropriate amount of payment and include a self-addressed stamped envelope for the return of documents. Note: Requests submitted without a return envelope will not be processed.
